1. What commissions are paid?
Group Soliciting Commissions are payable at a flat rate as follows:
4% for all EPO and PPO Plans.

Commission checks are drawn on the 10th of every month.

3. How do I get appointed with The PerfectHealth Insurance Company?
You must have a signed and dated copy of the following forms:
Producer Commission Agreement (32k, PDF)
Producer Information Sheet, Page 1 (74k, PDF)
Producer Information Sheet, Page 2 (6k, PDF)
Business Associate Agreement (58k, PDF)
W9 Form (32k, PDF)
We also need a current copy of your New York State Accident & Health Insurance License.
