Important Notice About Your
Health Plan
As you may know, PerfectHealth Insurance Company (PHIC) was merged into Group Health Incorporated (also known as GHI, an EmblemHealth company) on December 31, 2009. At that time, GHI assumed all rights, responsibilities, liabilities and obligations of The PerfectHealth Insurance Company (also known as PerfectHealth) under your contract or certificate of coverage. Please view the GHI Certificate of Assumption.
All former PerfectHealth group plans were discontinued by GHI on May 1, 2010. Unless your employer group chose to purchase other replacement coverage from GHI or another health insurance company before May 1, your plan was automatically replaced with the EmblemHealth ConsumerDirect plan underwritten by GHI. This EmblemHealth plan generally provides you with more benefits, services and advantages than you received under your former plan.
